Re: Replacement Health/Prescription ID Card

Dear Student:

In the past you were provided an Oman Student Health Plan ID card and a Merck Medco Prescription Drug card. This year Harrington combined the data from the two cards, and consolidated it in one card, which will be sent out this week.

Please destroy the old cards and when next receiving medical treatment or filling a prescription, please present this new ID card.

The only other change on the card is the implementation of a new Pre-Certification and Hospital Utilization Review vendor. This change resulted in a different toll-free number (877) 294-7839, which must be called when a hospital confinement is scheduled, or within 24 hours after an emergency admission to a hospital. The number is printed in the back of the card.

The back of the card also contains the PHCS network toll free number (866) 297-9105 and the web site address for provider information. We urge you to use the network providers to avoid any advance payments.

For prescription information call Medco Health Member Services toll free number (800) 903-8641. The number is also printed on the back of the card.
